Event Description
The customer reported to olympus, the evis exera iii xenon light source unit popped and blew up, smoke was coming out of it.The sales representative said to send this unit in as a preauction, in case it was affected too.The issue was found during reprocessing following a diagnostic colonoscopy.There were no reports of patient harm.Related patient identifiers: (b)(6).

The device is not expected to be returned for evaluation as the customer said the sales representative then checked the unit and said it was fine.The investigation is ongoing and follow-up with the user facility is currently being performed.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.

This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.New information added to the following fields: h6.Corrected fields: d4 a review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over (2) years since the subject device was manufactured.A definitive root cause could not be determined, and olympus was unable to identify the probable cause of the phenomenon from the investigation results.Olympus will continue to monitor field performance for this device.
