Event Description
It was reported that the brake failed.The patient presented with angina and required a rotational atherectomy procedure.During removal of a rotapro device while in dynaglide mode, the "burr vibrates" indicating a console speed issue.The advancer brake failed to lock on the guidewire and the guidewire couldn't be controlled forcing both the burr and wire out of the patient's body.The procedure was completed with the original device and no patient complications were reported.
