Event Description
The customer did not specify a problems.The sales representative reported to olympus that the device was not broken, but wanted to determine the possible cause of blue liquid coming from the scope during an uspecified procedure.Inspection and testing of the returned device found the front panel has blue liquid inside where the patient circuit board (pcb) is located, which can cause problems in the future.Blue liquid was also found inside the connector and air pump tubes.There were no reports of patient harm.This medical device report (mdr) is being submitted to capture the reportable malfunction found during evaluation.
 
Manufacturer Narrative
The device was returned to olympus for evaluation and the allegation of blue liquid coming from the scope was confirmed.The device evaluation also found the lamp had over 500 hours of use.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.

This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over (6) six years since the subject device was manufactured.Based on the results of the investigation, a definitive root cause could not be determined.However, it is likely that the phenomena occurred due to fluid invasion.Olympus will continue to monitor field performance for this device.
