Event Description
|
It was reported that the female connector of the minicap transfer set was broken and was unable to be disconnected from the patient line of an amia cassette.This occurred after peritoneal dialysis therapy.The patient had to cut the patient line to disconnect.The transfer set was replaced.There was no report of patient injury or medical intervention associated with this event.No additional information is available.

Additional information: h3, h6 and h10.H10: the actual device was not available; however, three (3) photographs of the sample were provided for evaluation.A visual inspection with the naked eye identified a separation between the female connector and main body.The reported broken female connector was verified.The cause of the separation was determined to be manufacturing related issue due to inadequate solvent to the insert chip.The reported condition of inability to disconnect the female connector was not verified.A batch review was conducted and there were no deviations found related to this reported condition during the manufacture of this lot.Should additional relevant information become available, a supplemental report will be submitted.
