According to the reporter, in a laparoscopic lung procedure, while in the lung, the device did not fire.The surgeon was able to press the green button but was unable to squeeze the handle.Another device was used to complete the case.There was no patient injury.

According to the reporter, in a laparoscopic lung lobectomy, the nurse could not load the reload onto the stapler.Another handle was used with the same reload to complete the case.There was no patient injury.Medtronic's initial evaluation of the incident device found that sheared teeth were visible on the firing rack due to thick tissue.
