Event Description
It was reported this was a mitraclip procedure to treat mixed mitral regurgitation (mr) with a grade of 2+ and a rotated heart. An ntw clip was inserted. However, after grasping, the patients blood pressure lowered and went into cardiogenic shock. The patient was stabilized after cpr was performed and medication was administered. Therefore, the clip was removed, and the procedure was discontinued.Mr remained at a grade of 2+.There was no clinically significant delay in the procedure. No additional information was provided.

Manufacturer Narrative
The device was not returned for analysis.A review of the lot history record revealed no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Based on available information, the cause of the reported cardiogenic shock cannot be determined. The reported hypotension and mitral stenosis (ms) appear to be cascading effects of the reported cardiogenic shock.Additionally, the reported patient effects of hypotension and mitral stenosis are listed in the instructions for use, and are known possible complications associated with mitraclip procedures. The reported medication required, and medical intervention (cpr) were results of case-specific circumstances. There is no indication of a product quality issue with respect to manufacture, design, or labeling.
