Event Description
Medtronic received information regarding a navigation system being used during a functional endoscopic sinus surgery (fess) procedure.It was reported that the surgeon was performing trace registration and was failing.The back of the head was cropped out and a similar trace pattern was used again and registration passed without issue.The system was also intermittently unresponsive.Software was reinstalled and the issue persisted.The probable cause was the solid state drive (ssd).There was no impact on patient outcome.

H3/h6: the software analysis was completed.As per the case description, the site was facing the issues registering the patient.However, after editing the model the similar trace pattern was used and registration passed without issue.Suggested that the site use the 3 point trace registration to avoid the patient orientation detection issues if any.The system was also unresponsive intermittently.However, the ssd will not be returned, so logs and archives will not be available.Without logs and exam archives, there is insufficient information to determine whether a software anomaly contributed to the reported behavior.Codes b17, c19, and d15 apply.Medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report.This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.Medtronic will submit a supplemental report if additional relevant information becomes known.
