The device was returned to olympus for evaluation and the customer's allegation of the device was reprocessed without ethylene oxide cap was confirmed.The device evaluation found the device was not reprocessed per the instruction manual.The bending section cover was broken because the cap was not put in correctly, resulting in air/water leakage.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
The customer reported to olympus, the cysto-nephro videoscope was reprocessed without ethylene oxide cap.The issue was found at reprocessing, there was no patient involvement.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation and to correct d9.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause could not be identified although it can presumed that it was due to the misuse of users.The event can be detected/prevented in accordance with the following instructions for use: instructions visera cysto-nephro videoscope olympus cyf type v2 olympus cyf type va2 olympus cyf type v2r chapter 7 cleaning, disinfection, and sterilization procedures olympus will continue to monitor field performance for this device.
