The device was returned to olympus for evaluation and the evaluation found evidence of intermittent failure in the front panel of the equipment, buttons are crystallized and do not allow them to work properly.The equipment was disassembled, internal boards were checked, and the equipment was cleaned internally.The equipment was assembled, and external cleaning was carried out.Functional tests were carried out again and the equipment was out of service.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
The customer reported to olympus, the evis exera ii xenon light source had intermittent failure in the front panel of the equipment.The issue was found during reprocessing after an unspecified diagnostic procedure.There were no reports of patient harm.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.A review of the device history record found no deviations, that could have caused or contributed to the reported issue.It has been over 13 years, since the subject device was manufactured.Based on the results of the investigation, it was not possible to determine, a probable cause for this phenomenon.Olympus will continue to monitor field performance for this device.
