Event Description
It was reported that there was a separation between the female connector (dark blue portion) and the main body of the minicap transfer set; further described as ¿trying to disconnect from manual bag, we were unable to disconnect as the dark blue twist on the transfer set started to untwist from the light blue¿.This occurred after peritoneal dialysis therapy when disconnecting from the manual bag.To resolve this issue, the transfer set was replaced.There was no report of patient injury or medical intervention associated with this event, however the patient was given prophylactic treatment.No additional information is available.

H10: the actual device was not available; however a photograph of the sample was provided for evaluation.The photograph was visually inspected and a separation between the female connector and main body of the twist clamp was observed.The reported condition was verified.The cause of the condition was determined to be a manufacturing issue.A batch review was conducted and there were no deviations found related to this condition during the manufacture of this lot.Should additional relevant information become available, a supplemental report will be submitted.
