Event Description
The customer reported to olympus, the evis exera iii xenon light source had minor cosmectic wear and tear with small dents on the front panel that did not affect functionaility.The issue was found during maintenance.No patient or procedure was involved.The device was returned for evaluation.During the device evaluation, it was found that there was a faulty scope socket.This medical device report (mdr) is being submitted to capture the reportable malfunction found during evaluation.

The device was not returned to olympus for evaluation.The investigation is ongoing and follow up with the user facility is currently being performed.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.

This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation and the device evaluation.Information added to the field: h3, h6.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 8 years since the subject device was manufactured.The device was returned, and an evaluation was completed for it.During inspection, olympus confirmed the reported event.Based on the results of the investigation, a root cause could not be determined.However, it is likely that scope detection was not working due to faulty scope socket.Olympus will continue to monitor field performance for this device.
