Event Description
According to the reporter, during a robotic laparo-assisted myomectomy, the clip applier jammed during use, so the clips were out of order and impossible to be removed.It was noted that the clip jammed from the start of use, clip jam impossible to remove and clip impossible to close.New forceps was used to resolve the issue.There was no patient injury.
