Event Description
The customer reported to olympus, that the uretero-reno videoscope cannot angulate sufficiently.The issue was found during reprocessing.There was no patient involvement.The device was returned and evaluated; the distal tip does not flex all the way.This medical device report (mdr) is being submitted to capture the reportable malfunction found during evaluation.
 
Manufacturer Narrative
The device was returned and evaluated.In addition to the malfunction documented in b5, the additional evaluation findings are as follows: there was a leak in channel at distal end; bending section was unable to angulate - angulation lever frozen; fluid and corrosion on control body; unable to check angulation due to frozen angulation lever; and the control knob movement was frozen and unable to move.The investigation is ongoing, a supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause of the reported event is unable to be determined.However, the cause of the event is likely due to leakage of the biopsy channel resulting in water invasion of control section and corrosion.The event can be detected by following the instructions for use (ifu) section: -inspection of the bending mechanism.The event can be prevented by following the instructions for use (ifu) section: -leakage testing of the endoscope.Olympus will continue to monitor field performance for this device.
