Event Description
During a ventricular tachycardia ablation procedure, a pericardial effusion occurred which required a pericardiocentesis.Ablation had been performed in the left ventricle and towards the end of the procedure, the patient became hypotensive.A transthoracic echocardiogram revealed a pericardial effusion for which a pericardiocentesis was performed to stabilize the patient. There were no performance issues with any abbott devices.

Manufacturer Narrative
The results of the investigation are inconclusive since the device was not returned for analysis.Further information regarding the procedure was obtained stating the power settings used during the procedure was between 40-50 watts, and point-by-point ablations were performed lasting up to 60 seconds. The tactiflex ablation catheter sensor enabled instructions for use (ifu) recommends ablation duration between 10-20 seconds for power settings between 40-50 watts.Due to unknown procedural conditions, we are unable to conclusively determine the cause of the reported pericardial effusion.Per the ifu, cardiac tamponade and pericardial effusion are known risks during the use of this device.
