Event Description
The customer reported to olympus that the switch-on mechanism of the on/off switch of the light source was defective.There was no patient harm associated with the event.
 
Manufacturer Narrative
The device was not returned to olympus for evaluation.The investigation is ongoing and follow up with the user facility is currently being performed.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.

This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.New information added to the following fields: d8, d9, h3, h4, h6, h10.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over eleven (11) years since the subject device was manufactured.The device was evaluated by the field service engineer.During the inspection, olympus confirmed the reported event, and the xenon lamp was also replaced.A complete evaluation was not possible because the scope was missing.Based on the results of the investigation, a definitive root cause could not be determined.However, it is likely that the on/off switch mechanism was defective and could have caused the failure of the power switch.Olympus will continue to monitor field performance for this device.
