Event Description
It was reported that during a port placement procedure, the blood return allegedly could not be drawn.It was further reported that the normal saline could not be flushed.There was no reported patient injury.

H10: as the lot number for the device was provided, a review of the device history records will be performed.The return of the sample is pending.However, photos were provided for review.The investigation of the reported event is currently underway.H10: d4 (expiry date: 01/2025) h11: section a through f ¿ the information provided by bd represents all the known information at this time.Despite good faith efforts to obtain additional information, the complainant/reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.

H10: manufacturing review: the device history records have been reviewed and this lot met all release criteria.There was nothing found to indicate there was a manufacturing related cause for this event.Investigation summary: the physical device was not returned for evaluation.Two photos were provided for review.The photo shows the clinician holding a syringe attached to the infusion set which punctures the powerport septum.Therefore, the investigation is inconclusive for the reported flushing and suction issues as no clear evidence of the reported flush and suction issues were provided for review.A definitive root cause could not be determined based upon the provided information.Labeling review: as the reported event did not allege a labeling or use related issue, a labeling review is not required.H10: b5, d4 (expiration date: 01/2025).H11: section a through f ¿ the information provided by bd represents all the known information at this time.Despite good faith efforts to obtain additional information, the complainant/reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.H3 other text : device not returned.
 
Event Description
It was reported that during a port placement procedure, after connecting the port seat to the catheter during the surgical implantation process, the blood return allegedly could not be withdrawn from the port and the normal saline allegedly could not be bolused.It was further reported that after withdrawal of the port from the body, the blood return and the bolus still could not be withdrawn.There was no reported patient injury.
