It was reported that on an unknown date, the retention pin inserters seemed stripped while trying to attach screw to driver.Would spin the mechanism to try and lock into the screw, but the threads would not engage with the screw.There was no patient consequence; the issue was noted postoperatively.A total of three drivers were affected.This report involves one retention pin for screwdriver w/ q c -hex 12mm/short/xl25.This is report 1 of 3 for (b)(4).
