The device will not be returned to olympus for evaluation.The investigation is ongoing, and a supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
The customer reported to olympus, the cysto-nephro videoscope was reprocessed using an oer-6 with a third party cleaning tube.The issue was found during reprocessing.There were no reports of patient harm.Related patient identifiers: (b)(6).
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.The device was not returned to olympus.However, based on the results of the investigation and customer¿s reported information, the probable cause is that the phenomenon occurred due to the handling methods of the users and not following instructions for use (ifu) with use of third-party component.The subject device manufacture date was not identified with unknown serial number; therefore, the device history record was unable to be reviewed.However, olympus only releases products to market that meet all manufacturing specifications and final product release criteria.The instruction manual for oer-6 identifies the following which could have prevented the phenomenon: - list for compatible cleaning tube no.3.Olympus will continue to monitor field performance for this device.
