E1 establishment name: (b)(6) hospital.The device was not sent to an olympus service center for evaluation.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
The customer reported the insufflation device shut down automatically during a therapeutic gynecological surgery.The procedure was completed using another similar device.There was no harm or user injury reported due to the event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's investigation.D8/d9, h3 were updated, and additional reportable findings are noted below.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over eight years since the subject device was manufactured.Based on the results of the investigation, the root cause of the reportable events could not be determined.In addition to the customer's reportable event, the following reportable malfunctions were also found during the device evaluation: it was found that the device generated an alarm and the front panel display disappeared occasionally due to faulty cr board.Lastly, the 1st regulatory unit((b)(6)) was damaged, resulting in insufficient gas feeding.Olympus will continue to monitor the field performance of this device.
