Event Description

According to the reporter, during a laparoscopic cholecystectomy procedure, at the time of clipping the cystic artery, the first clip applier was not forming properly formed clips.The clips were crossing over one another.Hence, a second clip applier was used from the same lot, and when the handle was squeezed, the clips were flying out.The clips fell into the patient's cavity and the surgeon knew how many clips had spring off in the clip applier.The clips were able to be retrieved.The surgeon did a washout to ensure nothing was left in the abdomen and an extra precaution.Another clip applier was used to resolve the issue.There was no patient injury.
