The device was returned to olympus for evaluation and the evaluation found that because the inner tube from the flow rate sensor was disconnected the flow rate was out of the standard value.The investigation is ongoing and follow up with the user facility is currently being performed.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
The customer reported to olympus, that during a therapeutic laparoscopic removal of biliary cystic there was no gas volume on the display of the high flow insufflation unit.The procedure was completed using a similar device.The device was returned for evaluation.During the device evaluation, it was found that the flow rate if out of the standard value.There were no reports of patient harm.This medical device report (mdr) is being submitted to capture the reportable malfunction found during evaluation.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.Since the device is more than 15 years old, the device history record was unable to be reviewed.However, olympus only releases products to market that meet all manufacturing specifications and final product release criteria.Based on the results of the investigation it was confirmed the flow rate display issue was due to a disconnected inner tube from the flow rate sensor.The cause of the disconnected tube was not established.Olympus will continue to monitor field performance for this device.
