Event Description
|
Philips received a complaint on the v60 ventilator, indicating that there was a pressure regulation high error code.The device was reported to be outside of use at the time of the reported problem.No patient or user harm reported.The remote service engineer (rse) spoke with the customer, who stated that they were receiving the pressure regulation high error code after replacing the flow sensor assembly (fsa).The rse performed troubleshooting with the customer and found that the proximal and outlet lines were switched when the fsa was replaced.The customer realigned the lines back to their proper orientation and tested the device.The proximal alarm triggered properly, and the customer confirmed that the device was operational.The investigation concludes that no further action is required at this time.If additional information is received the complaint file will be reopened.
