Device report from synthes reports an event in peru as follows: it was reported that two (2) imf screws broke during the procedure.The first screw broke while being inserted into the lower right jaw.The surgeon had successfully placed a screw in the upper right jaw just before.Then the surgeon placed two (2) screws in the left parallel areas without an issue.The surgeon went back and successfully removed the broken screw pieces from the lower right jaw area and inserted a new screw there.That screw also broke and the pieces were successfully removed.The surgeon noted the same pressure was used while inserting all the screws.The surgeon then drilled first the lower right jaw area and was able to insert a screw to the area with no issues.The surgery was completed successfully with no delay.This report is for an imf screw.This is report 1 of 2 for (b)(4).

Depuy synthes is submitting this report pursuant to the provisions of 21 cfr, part 803.This report may be based on information which depuy synthes has not been able to investigate or verify prior to the required reporting date.This report does not reflect a conclusion by fda, depuy synthes or its employees that the report constitutes an admission that the device, depuy synthes, or its employees caused or contributed to the potential event described in this report.A manufacturing record evaluation was performed for the finished article lot and no non-conformances were identified.Part:201.932, lot no: l634904, release to warehouse date: 27 oct 2017, manufacturing site: werk mezzovico.The product was returned to depuy synthes for evaluation.The depuy synthes team conducted a visual inspection of the returned device.Visual analysis of the returned sample revealed that the imf screw ø2 l12 sst has broken at me middle section of the body screw.No other issues were observed, the broken fragment was returned.A dimensional inspection was not performed due to post manufacturing damage.The observed condition of the device was consistent with a random component failure that may have been caused by exposure to excessive/unintended forces.The overall complaint was confirmed as the observed condition of the imf screw ø2 l12 sst would contribute to the complained device issue.There is no indication that a design or manufacturing issue has caused the complaint condition.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.If information is obtained that was not available for this medwatch, a follow-up medwatch will be filed as appropriate.
