The device was not returned to olympus for evaluation.The customer reported that the power switch was replaced per product instructions.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
The customer reported to olympus that the light source power switch could not be turned on.The issue was found during preparation for a cystoscopy procedure.The procedure was completed with a similar device with no delay.There were no reports of patient harm.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the results of the device evaluation and the legal manufacturer's final investigation.New information added to the following fields: h4, h6, h10.Corrected fields: b5, d9, h6(type of investigation).A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over eleven (11) years since the subject device was manufactured.The device was evaluated by a field service engineer and confirmed the power switch was damaged.The field service engineer replaced the power switch assembly.Based on the results of the investigation, a definitive root cause of the damaged power switch could be identified.Olympus will continue to monitor field performance for this device.
