Event Description
An olympus employee reported on behalf of a customer, the high flow insufflation unit kept bringing up errors.The event occurred during preparation for use.The original unspecified procedure was completed using a similar replacement device.There was no report of patient harm associated with this device.The device was returned for evaluation.During the device evaluation, the following reportable malfunction was identified: error log indicated multiple instances of error code 01 (irregularity with power voltage) and error code 05 (irregularity with backup data) were displayed.This medical device report (mdr) is being submitted to capture the reportable malfunction found during evaluation.
 
Manufacturer Narrative
The subject device was returned to olympus for evaluation.During inspection and testing, the allegation was confirmed due to an issue with the printed circuit board.In addition to evaluation in b5, the top cover had scratches.There was internal dust.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause of the event could not be determined although it can be presumed that the event occurred due to faulty main board.Olympus will continue to monitor field performance for this device.
