Event Description
|
Medtronic received a report that the react 71 catheter was found broken and leaked.The patient was undergoing surgery for treatment of a right internal carotid artery occlusion.The accessed vessel was the femoral artery.It was noted the patient's vessel tortuosity was moderate.It was reported that the right internal carotid artery was occluded, and the c2 end of the internal carotid artery was tortuous at 90 degrees.The solitaire stent was used to prepare the pull-out combination.When withdrawing, react 71 broke.The break was located at the hydrophilic coating at the distal end of the catheter.It was reported the catheter leaked in the distal section.The catheter was not inspected or tested prior to use.The leak was observed during use.The reported device and any accessory devices were prepared as indicated in the instructions for use (ifu).No patient symptoms or further complications were reported as a result of this event.Ancillary devices include a rebar 18 microcatheter.

H3: product analysis of react-71, lotno:b573190 found no damages with the react-71 catheter hub.The react-71 catheter body was stretched from ~17.5cm to ~13.0cm from the catheter distal tip.In addition, the react-71 catheter body was stretched at ~10.5cm from the catheter distal tip.No damages were found with the react-71 catheter distal tip.No breaks or separations were found with the react-71 catheter.Based on the device analysis and reported information, the customer¿s ¿catheter separation/break¿ and ¿catheter leak¿ reports could not be confirmed.The returned react-71 catheter was found stretched; however, no breaks or separations were found with the returned react-71 catheter.Medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report.This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.Medtronic will submit a supplemental report if additional relevant information becomes known.

Event Description

Additional information was received that the cause of the catheter leak was not determined.Drip infusion leaked and reduced suction efficiency.The catheter did not break into multiple pieces and it was all removed from the patient.It was noted that there may be some drip leakage from the leak point that came in contact with the patient.There were no additional medicinal or surgical interventions required as a result of the catheter break or leak.
