|
It was reported that during a cryo ablation procedure, a system notice was received indicating that the safety system detected a compromised outer vacuum.The electrical umbilical cable was reconnected and replaced without resolution.The coaxial umbilical cable was replaced without resolution.The console was restarted without resolution.A demo balloon catheter was used for troubleshooting and the system notice did not appear.At that point, the balloon catheter was replaced which resolved the issue.Additionally, it was noted that the distal part of the mapping catheter was broken.The mapping catheter was replaced which resolved the issue. the case was completed with cryo. no patient complications have been reported as a result of this event.
