This report is being submitted retrospectively as part of internal review.The returned transmitter was investigated, and its battery was found to be defective.The issue was further investigated and addressed through a corrective and preventative action (capa).Since the transmitter was outside of warranty period, user was advised to purchase a new transmitter.Per data management sytem (dms), the user had been using the system with a new transmitter without any issue.

Senseonics was made aware of an instance where patient reported that after placing the transmitter in the charging cradle for charging, it started to smell like smoke after 2 to 3 minutes.When patient tried to remove the transmitter out of cradle, it was bit hot.After few minutes when he finally took it out, he saw a crack in the middle of the button.
