He device was returned and evaluated.During the evaluation, it was determined that deterioration of the angulation mechanism due to rust.The investigation is ongoing, and a supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
The customer reported to olympus, that the ureteto-reno videoscope was leaking.The issue occurred during reprocessing of the device.The device was returned for evaluation.During the device evaluation, deterioration of the angulation mechanism by rust was found.There were no reports of patient harm.This medical device report (mdr) is being submitted to capture the reportable malfunction found during the evaluation.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause of the reported event is unable to be determined.However, the likely cause of the reported event is due to the a-wire was stuck in the a-coil due to corrosion caused by fluid invasion on the insertion section.The suggested event is detectable at the inspection prior to use that is stated in operation manual 3.2 inspection of the endoscope.Olympus will continue to monitor field performance for this device.
