Event Description
Philips received a complaint from customer biomed reporting the v60 ventilator failed to produce the appropriate audible alarm when a high priority alarm occurred.The device was in clinical use.There was no report of harm; no adverse outcome to patient care or therapy resulted from this event.The device did not meet specifications for intended use and was removed from service.A philips remote service engineer (rse) evaluated the issue with the biomedical engineer (bme) and confirmed the device was connected to a nurse-call system and alarmed correctly.The bme provided the rse with a video of the reported alarms.The rse advised the customer that the video indicates the alarm was silenced.The icon in the upper left-hand corner counts down from 2 minutes when the alarm is silenced and reactivates if the alarm condition has not been cleared.The bme retested the device and confirmed it to be operating per specifications.No failure was identified.The investigation concludes that no further action is required at this time.If additional information is received the complaint file will be reopened.
