The device was not returned to olympus for evaluation.An olympus field service engineer (fse) was onsite troubleshooting the issue.It was determined that the internal water filter had become clogged, causing a low water flow.The fse assisted the customer with the endoscope reprocessor external filter change as well as the external wall filters.After the filters were successfully changed, the pressure and flow rate increased.The investigation is ongoing and follow up with the user facility is currently being performed.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
A customer reported to olympus that the endoscope reprocessor washer cycle time had increased.There was no report of patient harm associated with this event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 8 years since the subject device was manufactured.Based on the results of the investigation, a definitive root cause could not be established.The suggested phenomenon was presumed to have been due to the water filter clogging due to temporary deterioration in water quality used at the user facility, and therefore the water supply time became longer than usual.The user can detect and prevent the suggested event in accordance with the following instructions for use (ifu): chapter 7 routine maintenance 7.2 replacing the water filter (maj-824 or maj-2318) replace the water filter at least once a month to prevent contamination of the rinse water.The water filter should also be replaced whenever an error code indicating water supply insufficiency [e01] is displayed.The water filter should be replaced by following the flow shown below.Olympus will continue to monitor field performance for this device.
