Event Description
|
According to the reporter, during the laparoscopic sleeve gastrectomy , on first firing at the transection of stomach, the suture at end of reload never got cut by the knife blade. To resolve the issue, the surgeon had to cut the suture at distal end of reload with shears. There was no patient injury.Medtronic's initial evaluation of the incident confirmed that the system terminated firing at the determined end stop zone as intended by the software but prior to the distal suture being released. The cause of the system stopping the firing prior to releasing the distal suture could not be determined.
