Event Description
It was reported that on (b)(6) 2023, a foley catheter was inserted and fixed with 7cc of distilled water.On (b)(6) 2023, it was discovered that the catheter had come out and the balloon was found deflated.There was no balloon damage, but water leaked from the connection where fixed water was injected.

Manufacturer Narrative
The reported event was inconclusive because no sample was returned.A potential root cause for this failure could be due to "imperfection in balloon due to process".A review of the device history record did not show any problems or conditions that would have contributed to the reported issue.The instructions for use were found adequate and state the following: "visually inspect the product for any imperfections or surface deterioration prior to use." section a through f - the information provided by bd represents all the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.
