The device was returned to olympus for evaluation and the evaluation found grip, switch box, universal cord had a scratch; air water cylinder, suction cylinder had no color; sheet holder unit had corrosion due to water leakage; guide pin of electrical connector had corrosion; objective lens, light guide lens had a crack; connecting tube was snaking.The investigation is ongoing and follow up with the user facility is currently being performed.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.

Event Description

It was observed that during the device evaluation, the duodenovideoscope exhibited white foreign material in air water tube, air water cylinder; foreign objects around adhesive of light guide lens, connecting tube; distal end was found cracked.There were no reports of patient involvement.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issues.Based on the results of the investigation, the root cause of the event could not be determined although it can presumed that it was due to irregular stress was applied to the distal end during device handling by the user.The root cause of the foreign material also could not be determined.The first event can be detected/prevented in accordance with the following instructions for use: "an insufficiently reprocessed endoscope and/or accessory may pose an infection control risk to the patients and/or operators who touch them." olympus will continue to monitor field performance for this device.
