Event Description
Philips received a complaint from the customer, reporting that the v60 ventilator displayed a 122d alarm message: cannot reach target flow.The device was in clinical use when the issue occurred, the device was removed from service.No patient or user harm reported.A philips remote service engineer (rse) noted that the customer's v60 ventilator displayed a 122d alarm message: cannot reach target flow.During the call with the customer, it was reported that while the device was in high flow therapy (hft) mode, the device displayed a "cannot reach target flow" alarm.A biomed reviewed the event log and noted error code 122d.The customer requested a bench repair be performed on the device.The investigation is ongoing.

Service engineer (se) evaluated the device, and reported that the issue was not duplicated.The se then reviewed the event log and confirmed that the 122d error was logged in the records.The se noted that the 122d was related to the high flow therapy (hft) otpion, and the device would be serviced.The repair is pending.
