Event Description
It was reported, the cysto-nephro videoscope had reprocessing issues.The biomedical engineer (bme) at the facility emailed technical assistance center (tac) as he had concerned about scopes being improperly processed and transported.There were no reports of patient harm.
 
Manufacturer Narrative
The olympus endoscopic support specialist (ess) called and spoke to the biomedical engineer (bme) and provided the bme with reprocessing information from the instruction for use (ifu).The ess asked the bme is they would like onsite training to be provided but the bme declined and states he felt confident and had more understanding of the workflow of the cysto-nephro videoscope and the gi colon scope and transportation recommendation from the ifu's.As part of the investigation into this report, an olympus ess has been dispatched on-site to assess the reprocessing practices at the user facility.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.

This report is being supplemented to provide additional information based on the legal manufacturer's investigation and correction to d4.The device history record was unable to be reviewed for this device.However, olympus only releases products to market that meet all manufacturing specifications and final product release criteria.Based on the results of the investigation, the root cause of the insufficient/incorrect reprocessing of the scope was not determined.Olympus will continue to monitor field performance for this device.
