The device was not returned to olympus for evaluation.During troubleshooting with olympus technical assistance center (tac) the customer tried to shut the drawer and it was not pushed in.The customer tried to stop and clear the load disinfectant solution (lcg) process and it is locked up on the oer-elite endoscope reprocessor screen.The customer tried cycling power, creating a high-level error and the program continues to respond with load lcg, the customer chose start and the draw is bound and will not close.Equipment was repaired and verified according to original equipment manufacturer (oem) instructions.Fixed disinfectant drawer lock and was tested successfully.Confirmed the procedure by using the service and repair checklist which was attached to this service request.Software attributes were verified and confirmed.Electrical safety was not performed because side panels were not removed.The investigation is ongoing and follow-up with the user facility is currently being performed.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
It was reported, the endoscope reprocessor drawer was stuck and the disinfectant bottles could not be installed.The issue occurred during reprocessing.There were no reports of patient harm.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, root cause could not be identified although it can be presumed it was due to the drawer not being pulled out for the lock of the acecide drawer was broken.Olympus will continue to monitor field performance for this device.
