Event Description
It was reported, that while using the light source, the connection was too difficult to the front socket.The issue occurred during a therapeutic gastric sleeve procedure.The procedure was completed with the same device, without delay by firmly re-setting the light guide so the high mode would turn on.There were no reports of patient harm.
 
Manufacturer Narrative
The device was returned to olympus for evaluation and the evaluation found the scope socket was faulty, the locking mechanism and scope detection slide were non-functional causing the front panel to blink intermittently.The device failed full inspection due to non-olympus lamp replacement and faulty scope socket.The investigation is ongoing and a supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 9 years since the subject device was manufactured.Based on the results of the investigation, the root cause for the event could not be determined.However, it is likely that image was not displayed, and front panel blinked because locking mechanism and cope detection slide did not work.Olympus will continue to monitor field performance for this device.
