Event Description
|
It was reported, the reprocessor incoming water had a very poor flow, just trickling into the basin.After troubleshooting with olympus technical assistance center (tac); pre filters had to be changed and customer reported that they had changed the pre filters that morning and tac suggested that the pre filters may have been installed incorrectly.The customer reseated the pre filters and ran a drain disinfectant solution counter (lcg) function and noted that the water dispensed into the basin at a normal level and worked normally.The issue was resolved.There were no reports of patient harm.

Updated fields: h6 this report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause was user error.The customer did not properly replace the water filter.After reseating the replacement filter, the reported issue was resolved.The following information from the instructions for use (ifu) pertains to this event: "chapter 7 routine maintenance 7.2 replacing the water filter (maj-824) replace the water filter at least once a month to prevent contamination of the rinse water." olympus will continue to monitor field performance for this device.
