Event Description
Medtronic received information regarding a navigation system being used outside of a procedure.It was reported that the instruments were not tracking.The navigation patient tracker and instruments were not showing up in the tracking window.This was a new system.The site verified instruments were in the instrument screen, and verified tracking details displayed emitter and electromagnetic navigation interface unit (axiem) were green.The site verified the emitter was close to both the tracker and instruments, and verified the emitter and axiem were seated correctly (site unplugged and replugged in).The site tried different ports on the axiem (the 2 middle rows of the axiem were showing orange). site replaced both instruments with the same outcome, and ran diagnostic tool and output didn't state any faults. site did not hear any noise from the emitter when holding their ear to it and confirmed they were on the registration screen.The most likely / suspected cause of the issue was the emitter, axiem, or metal interference. there was no patient involvement.

Event Description
Additional information was received.It was reported that the procedure being performed was a functional endoscopic sinus surgery (fess.) there was a surgical delay of approximately 45 minutes while troubleshooting.The surgeon opted to abort the use of the navigation system, and the procedure was completed without the use of medtronic hardware.There was no reported impact on patient outcome.

H3/h6: product 97335521 was analyzed by mistake and was not replaced - reported in previous report.Product 9735825 was analyzed.The returned eminterface was bench tested and it was found that the led's on ports three and four constantly stay amber.The remaining ports show connected in lat but the instruments do not track.Codes b01, c02, and d02 apply.Medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report.This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.Medtronic will submit a supplemental report if additional relevant information becomes known.
