Event Description
It was reported that during an irreversible electroporation ablation procedure to treat atrial fibrillation (a fib) using a farawave catheter the guidewire became stuck in the guidewire lumen. After attempting ablation on the right inferior pulmonary vein (ripv) they attempted to retract the guidewire but found it to be stuck. They removed the catheter and guidewire through the sheath. They were able to remove the guidewire from the catheter and observed tissue in the tip of the guidewire lumen. They exchanged the catheter, and the issue was resolved. The procedure was then completed with no patient complications. The catheter is expected to be returned for analysis.
