Event Description
This report is to advise of degradation of the paddle as exposed wires.
 
Manufacturer Narrative
One bi-directional, curve d-f, sensor enabled, advisor hd grid mapping catheter was received for evaluation.Visual inspection revealed the pellethane tubing on the paddle was degraded and flaked off exposing the wires and nitinol frame below.Electrode 13 read as an open circuit.Inspection revealed conductor wire 13 had been fractured proximal to the weld joint, consistent with the damage, open circuit detected and the reported signal issue.Electrodes 1-12 and 14-18 displayed acceptable resistance values.The device history record was reviewed to ensure that each manufacturing and inspection operation was performed.The cause of the fractured conductor wire could not be conclusively determined.However, it is possible the degradation of the paddle is due to the storage conditions of the device.The product instructions for use (ifu) states the catheter packaging is designed to prevent crushing of the product, to minimize product exposure to the atmosphere, and to provide for aseptic product transfer.It is recommended that the product remain in the unopened package until time of use.The product should be stored in a cool, dry, and dark location.
