It was reported that a mitraclip procedure was performed to treat degenerative mitral regurgitation (mr) with a grade of 4.A mitraclip xt was prepared per the instructions for use (ifu) and inserted without issues.However, while in the valve, it was observed that the posterior gripper was not functioning as intended.Troubleshooting was performed, but the issue was unable to be resolved.Therefore, the clip was removed and replaced.The procedure was completed with two clips implanted.The mr was reduced to trace.Post procedure, two attempts to drop the grippers were performed but it was not successful.There were no adverse patient effects and no clinically significant delay in the procedure.

All available information was investigated, and the reported single gripper actuation issue was confirmed via returned device analysis.Additionally, it was observed that the gripper line was caught on the frictional element (fe) of the gripper arm.A review of the lot history record revealed no manufacturing nonconformities issued to the reported lot that would have contributed to the reported event.A review of the complaint history identified no other complaints from the lot.Based on the information reviewed and the analysis of the returned device, the reported single gripper actuation issue was due to the gripper line being caught on the fe.However, a cause for how the gripper line became caught on the fe could not be determined.There is no indication of a product issue with respect to manufacture, design or labeling.
