Event Description
It was reported that one day post implant the patient was feeling pain on their side and heard an audible noise from their cardiac resynchronization therapy defibrillator (crt-d).Upon review it was noted that there were several right ventricular (rv) lead impedance alerts including for the pacing, svc defibrillation and rv defibrillation coils.It was also noted that there were high left ventricular (lv) lead thresholds and lv impedance was high.The patient was brought back to the lab and it was determined that the rv lead was not properly seated in the crt-d header.The pocket was reopened and the lead was appropriately connected to the device. no further patient complications have been reported as a result of this event.
