It was reported that during a procedure to treat paroxysmal atrial fibrillation a farawave catheter was selected for use, however, the catheter was constrained in a small area (searching for ripv) and difficult to deploy.After several manipulations and to resolve the issue, the physician tried to reinsert the farawave back in the fardrive, still, it was very difficult and when he finally succeeded in doing it, it was noticed that the farawave had two twisted splines.It was noted that the catheter was difficult to remove from the patient.It was not possible to move the splines manually in the correct position, therefore it was decided to replace the device, and the issue was resolved.The procedure was completed.No patient complications were reported.The device is expected to be returned for laboratory analysis.
