Event Description
It was reported that this system with a right ventricular (rv) lead and an implantable cardioverter defibrillator (icd) recorded events with noise from far field sensing of atrial flutter on the rv channel.The patient had a recent pulse generator changeout, therefore a lead pull back was suspected but not confirmed.Also, an x ray analysis was recommended.There were inappropriate shocks and anti-tachycardia pacing delivered but the rhythm was actually converted.There were pauses and at least one for more than two seconds.The rv lead and the icd remain in service.No additional adverse patient effects were reported.
