The device was returned and the evaluation found that there were connectivity issues with the socket in the front of the unit, this was due to failure of the video connector socket.Should additional relevant information become available, a supplemental report will be submitted.
 
Event Description
It was reported, that the video system center had a connectivity issue with the socket in the front of the unit.When the scope was connected to the front port connector, the video would go on and off when flexing the connection at the port on the processor.The issue occurred during preparation for use.The procedure was completed using a similar device.There was no delay reported.There were no reports of patient harm.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.Corrected fields: e2, e3, g2 and d4 serial number (b)(6).A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 12 years since the subject device was manufactured.Based on the results of the investigation, a definitive root cause could not be determined.Olympus will continue to monitor field performance for this device.
