Event Description
It was reported that the gastrointestinal videoscope was insufficiently/incorrectly reprocessed for the next procedure.There were no reports of patient harm associated with the event.
 
Manufacturer Narrative
The device was not returned for evaluation.However, an olympus endoscopy support specialist (ess) visited the site to provide a reprocessing in-service for the gif-xp190n scope.The staff at the site were trained on the proper way to care for and reprocess the scopes in accordance with the ifus.The investigation is ongoing, a supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, a definitive root cause of the reported videoscope being insufficiently/incorrectly reprocessed could not be determined, however, it is believed that there was a difference in understanding of equipment handling and reprocessing procedures between olympus' recommendations and the facility; an in-service has been conducted for the facility staff by an olympus endoscopy support specialist (ess) regarding proper device handling and reprocessing.The event may be detected/prevented by following the instructions section below: gif/cf/pcf-190 series reprocessing manual chapter 5 reprocessing the endoscope olympus will continue to monitor field performance for this device.
