Event Description
During a ablation a polarxfit catheter was selected for use.The patient experienced phrenic nerve injury.A double stop was performed at 124 seconds due to decrease of compound motor action potential (cmap) during cooling.It was reported the phrenic nerve movement did not recover during the procedure, but the ri was decided with radio frequency ablation procedure to complete the procedure.The catheter is not expected to be returned as it was disposed of by the site.
 
Manufacturer Narrative
It was indicated that the device will not be returned for evaluation.If there is any further relevant information obtained, a supplemental medwatch will be filed.
