The device was returned and the evaluation found the main lamp does not light.It shift into spare lamp due to defective converter; and the power switch was found stuck, and cannot be pushed, and lens based failure.Should additional relevant information become available, a supplemental report will be submitted.
 
Event Description
It was reported, the light source had a main lamp that does not light automatically, light output not coming on; and the power switch was found stuck, and cannot be pushed.The issue occurred during general routine inspection.There were no reports of patient harm.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over nine (9) years since the subject device was manufactured.Based on the results of the investigation, a definitive root cause could not be determined.However, it is likely that the main lamp is not lit due to defective converter, and optical output cannot be obtained because it was found that the lens base is damaged.Probable cause for power switch sticks could not be confirmed.Olympus will continue to monitor field performance for this device.
